Completion of Intake Packet

Obtain an intake packet at the Office of CalWORKs Education and Training or from a CalWORKs Counselor at the campus you wish to attend. An intake packet will have the following documents:

  • CalWORKs Education and Training application form.
  • Release of Information form.
  • CCSF Admission Applications: (you will be required to complete both a credit and a non-credit application form.
    • credit classes, you must complete a Credit Application.
    • non-credit classes you must complete a Non-Credit Application.
  • Board of Governors Fee Waiver application form.
  • EOPS application form.
  • Work Study application form.
  • Other documents may also be included.
