We have been advised by the Company that the annual pension statements
will be sent out at the end of June with a target delivery date
to employees of July 13 , 2000 .
We also advised the pension buy back information would be mailed
out about the same time, but in a separate package.

Brothers:
This is to inform you of the procedure regarding the application for the Group Insurance Employee Paid Life Insurance.
The optional Life Insurance may be purchased up to a maximum of $250,000 in multiples of $10,000. The monthly premiums are indicated in your table per thousand dollars of coverage (age 35 to 39 at $100,000 would have a monthly premium of $8.00 deducted from paycheque $100,000 x $0.08/1000). The response from Mr. Wheeler, Benefits and Job Income Security Specialist states that a medical examination may not be required as the medical information required is part of the application form which will provide evidence of insurability. On review by National Life of Canada, if additional medical information is required over and above what is requested on the application form, it would be filled out by the employees physician and any cost for the information would be borne by the employee.
