For our referral agencies, we would ask you to note the following:
Whilst our volunteers are visiting ever more clients, we also are receiving increasing numbers of referrals. We have amended our criteria and we can only meet the needs of the most socially isolated clients. Whilst this will mean some clients will not be accepted for a service, we hope that those who are most socially isolated, will be seen quicker.
You can refer a suitable person to the Scheme using the referral form below. You can and fill it in by hand and post it to us, or fill it in on your pc, save it to My Documents and attach it to an e-mail.
Please be certain that the client meets our criteria, in that:

They are 19 years of age or older

They are housebound as a result of PHYSICAL or sensory impairment or illness

They are socially isolated in that they go out once or less per week (other than medical appointments) AND receive one visit or less per week from a friend, visitor or relative
Please ensure that you have agreed with the person that you pass on their details and that they want a volunteer visitor from us. Assessing clients who do not meet our criteria or who do not want a visitor, wastes time which could be used to help someone who does need and want our services.
As a matter of professional respect and duty of care, we also expect that you will inform us of any infectious diseases or dangerous/inappropriate behaviours exhibited by the client.
