Please follow the procedures in processing your application for TIPQCAA membership:

1. Fill-out the form (refer to the sample form). Use black ink. For updates re contact no./s, home and/or provincial address, work information, etc., please email the details at tipqcaa@hotmail.com. 

2. Paste one copy 1x1 ID picture on the form.

3. Write your full name at the back of the 2 copies 1x1 ID picture. Place 2 copies in a small envelope and attach to the form.

4.  Photocopy your Receipt (TIP or TIPQCAA Receipt) and attach to the form. Write the Receipt no. and the kind of Receipt (TIP or TIPQCAA) issued to you on the box at the lower part of the form.

5.  Please drop your MEMBERSHIP FORMS at our dropbox with complete requirements.


***Application of those who do not have proper requirements will not be processed.

***For March 1985 - October 2004 graduates, please text or call
TIPQCAA Secretary Minnie Baldres at 0920-8140783 for payment of Membership Fee.
Receipt will be issued upon payment.


We accept check payments (payable to TIPQCAA) for Membership Fees / Donation / Sponsorship / Solicitation
Requirements:
1.  Filled-up TIPQCAA Membership Form
2. 3 copies 1x1 ID picture
3. P300 Membership Fee or  TIP/TIPQCAA Receipt


*** Your Alumni Membership Card or Alumni Membership Fee Receipt (TIPQC or TIPQCAA) will serve as a gatepass to TIPQC Campus and Library.
