2. Read and follow the
Church
School Procedure. This will explain all the forms that are needed
to be filled out. They are available here to print off. This way you
can read them and fill them outbefore I meet with you. (If you are not
able to print them off, I will have forms with me.)
3. Read and sign the
Statement of
Faith.
4. Complete and return the
Registration.
5. Read and sign the
Church School
Disclaimer.
6. Complete and return the
Teacher
Application.
7. Complete one Course of Study for each student. (Use either an
elementary
or
high school form for appropriate
grade.)
8. Since high school is based on credits, please list any courses already
completed on the
High School Credit
Accumulation Chart. This information will be checked and verified
when thestudent's records are received. It will also give you an idea
of what further courses are needed.
9. If you have any children that will be 7 years old by September 1
of this year, you will need to complete and return a Church School Enrollment
Form for
each child. I will have this form for you when I meet
with you. This form needs to be filed with the superintendent of education
in your school district by the 5th day after school begins.
10. If your child attended another school before your enrollment in
Christian Fellowship School, you may go to the school in person to request
your child's transcripts or complete a
Student Record Release Form for them.
If you obtained the records directly, be sure to send us a copy. Otherwise,
return the signed
Student Record
Release Form so we can request your child's transcripts. Please
be sure to include the complete mailing address of the school.
11. If this is your first time to enroll in Christian Fellowship Schoool,
the enrollment fee is $65 made out to Christian Fellowship School. (This
includes a one time charge of $35 per family orientation fee and a $30
per family per year registration fee.) Next year when you register,
there is only a $30 per family registration fee charged if all records
of the previous year, registration papers, and fees are turned in by
August 1. (The school year ends June 10, so everything is due in by
the following August 1.)
Any family that does not have records, registration,
and fees in by August 1 in NOT guaranteed acceptance into Christian
Fellowship School for the following school year.
12. Since parents are teachers in the school, as administrator, I must
be able to contact each family by phone during the school year. (August
15 to June 10) This would preferably be your own phone, but it may be
a phone at someone else's house. If the number is not your phone, the
family that has the phone must be able to easily contact you so that
you can return my call promptly. If at any time I cannot reach you by
phone (phone disconnected, moving, etc.),
your children will be automatically
expelled. So you must keep me notified of your change in phone number.
It is not possible for me to keep you legally safe if I can't reach
you. Send changes by e-mail, if at all possible, otherwise, call.
