Transition refers to planning and services that are needed to prepare
youth with disabilities for moving smoothly from school to adult living,
learning, or earning roles in the community after leaving secondary schoool.
In New York City, transition components are built into the student's Individualized
Education Program (IEP). All youth with disabilities, aged 14-21
must have transition components in their IEPs. To learn more about

What is a Transition Linkage Coordinator?
The Transition Linkage Coordinator or TLC is the person in your child's
high school who can make certain that the Transition goals are created
and who your child can speak with regarding life after high school.
