Goals of Transition

What is transition?

Transition refers to planning and services that are needed to prepare youth with disabilities for moving smoothly from school to adult living, learning, or earning roles in the community after leaving secondary schoool.  In New York City, transition components are built into the student's Individualized Education Program (IEP).  All youth with disabilities, aged 14-21 must have transition components in their IEPs.  To learn more about transition, click on the link to  New York State Department of Education Transition site  which is a comprehensive resource of information regarding transition throught the State of New York.  That site is external --  a site that is not part of the Queens Parents Transition site -- so you will have to X out of the site in order to come back here.
 

What is a Transition Linkage Coordinator?

The Transition Linkage Coordinator or TLC is the person in your child's high school who can make certain that the Transition goals are created and who your child can speak with regarding life after high school.

Two good sites to visit are the New York State Education Department Transition site and the Queens High School Office Parents' Center site.  These are external sites, which are not a part of the Queens High School Transition Parents' Site, so you will have to "X" out on the top right-hand side to get back to this site.
