MEMBERSHIP PAPERWORK:  What we need to get your young ones swimming!!

• MEMBERSHIP AGREEMENT AND BOOSTER CLUB REGULATIONS:  Please fill out completely so our records are intact. These forms are an agreement that you will support the booster club and adhere to the rules and guidelines set forth.

• MEDICAL INFORMATION FORM:  This form provides medical/insurance information for each swimmer.

• MEDICAL POWER OF ATTORNEY:  This form must be completed and notarized and in the coaches possession at beginning of initial two-week trial period. The Hickam Base Legal Office can notarize the form. Include the Hickam Hurricanes Coaches and the location of the pool (Pool #1, Hickam AFB). This entitles your child to immediate medical attention at team practice.

Pick the form below up on deck as it is a carbon copy type that is turned in to the national organization.

• U.S.S. REGISTRATION FORM:  This registers the swimmer with the national organization for swimming as an athlete member. Liability coverage is provided during U.S.S. sanctioned swim meets and team practices. This is very important and is required for your child to swim with the Hurricanes. The fee is payable with your initial start up costs and thereafter billed for annual registration. You need to return this form with a copy of your swimmer’s birth certificate.
